Care Management

A care manager - a master's level Social Worker  - will help you to evaluate the care needs of the older or disabled adult, and devise a cost-effective care plan consistent with the requirements and wishes of all concerned. A care manager can be particularly helpful when family members live at a distance or are not fully able to provide the planning, coordination, direct delivery or monitoring of needed services.

The Social Worker/Care Manager can provide:

  • Home visits
  • Counseling
  • Monitoring of in-home help
  • Crisis intervention
  • 24-hour on-call availability
  • Advocacy for, and location initiation, coordination and monitoring of community services such as home delivered meals, alternative housing, medical care, transportation, shopping, housekeeping and recreation
  • Ongoing liaison with family members
